Registration & Tuition Subsidy



STEP 1 - APPLY FOR THE CMSI TUITION SUBSIDY

If you are SELF-FUNDED (paying for the course out-of-pocket), skip to STEP 2
  1. To register for the CMSI Tuition Subsidy:
    1. You must register online for the subsidy Please note: This information is sent to the Office of Math and Science and NOT to the SESAME Office. This is why you must register with the SESAME Office separately.
    2. Download and complete the CPS Participation Contract. Complete Sections I and IV on page 7 (Section IV requires your principal's signature), and Section V on page 9. Make a copy of pages 7-9 for yourself.
    3. Download and complete the Educational Release Form. Make a copy for yourself.

  2. Mail the Contract and Release forms to:

  4. PLEASE NOTE: The SESAME office cannot register a participant until they are approved for the subsidy



STEP 2 - APPLY WITH THE SESAME PROGRAM

  1. Download and print out the SESAME Registration form (PDF file)

    2. (Don't have Adobe Reader? Download it for free here)
    1. Complete all sections of the SESAME Autumn Registration Form
    2. Verify whether you are applying for the CMSI Tuition Subsidy or if you will be SELF-FUNDED
    3. Include up to three different class choices (you will be placed into your first choice unless it has been filled)

  2. Fax your registration form to the University of Chicago, Mathematics Department office with
